Transferring records from a ForHealth practice
This applies if you are:
- Requesting records held in relation to you ( Ages 14 years and over)
- Requesting medical records for a child aged 13 years and under (parent or legal guardian)
- Requesting records for a deceased patient (authorised Executor)
- Guardian/Attorney requesting records on behalf of a patient
- Requesting records to be sent to a Medical Centre not owned by Forhealth
What we need to process your request
- A completed online request form with Signed consent and proof of identity (which will require your signature and consent, as well as proof of identification)
- payment of an administrative fee (our team will email or post you an invoice)
- please allow up to 5 weeks for processing

Do you Bulk-Bill?
Yes, Elizabeth Medical & Dental Centre offers 100% Bulk Billing for all GP consults. This means that if you have a valid Medicare card, there is no out-of-pocket cost for your GP consultation.
Note: Some procedures or services not covered under standard GP consultations may attract a fee.
What is Bulk Billing?
Bulk Billing means that the cost of your consultation is fully covered by Medicare, so you don’t need to pay upfront or claim anything back. Simply present your valid Medicare card at the time of your appointment.
Do any fees apply?
While all standard GP consults are Bulk Billed, private fees may apply for:
• Procedures not covered by Medicare
• Patients without a valid Medicare card
• Allied Health services
• Dental care
• Specialist consultations
• WorkCover, CTP claims, and other non-standard services
We’ll always let you know in advance if any additional fees apply.
How do I pay for non-GP services?
For services that aren’t Bulk Billed:
• All fees are payable in full on the day of your appointment
• We accept EFTPOS and major credit cards
• Dental services: Payment plans available via Afterpay and Zip
• Claims can be submitted on your behalf for eligible Medicare services
What if I don’t have a Medicare card?
If you don’t have a valid Medicare or concession card, you’ll be charged a private fee and won’t be eligible for a Medicare rebate.
